
GP Chronic Condition Management Plan
Medicare Physiotherapy/Chiropractic for Chronic Conditions

The GP Chronic Condition Management Plan (GPCCMP), previously commonly known as an EPC (Enhanced Primary Care) or CDM plan, allows eligible patients with chronic medical conditions to access Medicare-supported allied health services, including physiotherapy and chiropractic care.
If you have an ongoing condition such as chronic back pain, neck pain, arthritis or another musculoskeletal condition, your GP may recommend physiotherapy or chiropractic care as part of your Chronic Condition Management Plan.
Eligible patients can access up to 5 individual allied health services per calendar year, shared across the allied health services recommended by their GP.
Who Is Eligible for a Chronic Condition Management Plan?
You may be eligible if you have a chronic or terminal medical condition that requires ongoing management.
A chronic condition is generally one that has been, or is likely to be, present for at least 6 months.
Conditions that may benefit from physiotherapy include:

Arthritis and joint pain

Persistent musculoskeletal injuries

Knee, hip or ankle conditions

Reduced mobility and physical function

Ongoing pain following injury or surgery

Other long-term conditions affecting movement and mobility
Your GP will determine whether you are eligible and whether physiotherapy or another allied health service is appropriate for your condition.

How Does the Chronic Condition Management Plan Work?
1. Visit Your GP
Speak with your GP about your condition and whether a GP Chronic Condition Management Plan (GPCCMP) is appropriate for you.
If physiotherapy is recommended as part of your management plan, your GP can provide a referral for allied health treatment.
2. Book Your Physiotherapy/Chiropractic Appointment
Once you have your referral, contact our clinic to arrange your appointment.
Please bring your GP referral and Medicare card to your appointment.
3. Receive Physiotherapy/Chiropractic Treatment
Your physiotherapist/chiropracter will assess your condition and develop an individual treatment plan based on your symptoms, mobility and goals.
Treatment may include:

Manual therapy

Exercise therapy and rehabilitation

Strengthening and mobility exercises

Pain management strategies

Posture and movement advice
-
Home exercise programs

Education and self-management strategies
How Many Physiotherapy/Chiropractic Sessions Can I Have?
Eligible patients can access a total of up to 5 Medicare-supported individual allied health services per calendar year.
These 5 services are shared between the allied health professionals recommended as part of your care.
For example, your GP may recommend:
5 Physiotherapy sessions
or
3 Physiotherapy sessions + 2 Chiropractic sessions
depending on your individual health needs.
Your GP determines which allied health services are appropriate for your condition.

How Long Is My Referral Valid?
For referrals issued under the current GP Chronic Condition Management arrangements, your referral is valid for the period specified by your GP.
If no timeframe is specified, the referral is generally valid for 18 months from the date of your first allied health service under that referral.
The annual Medicare service limit still applies.
